Anticoagulants are moving to the frontline in some patients with COVID-19 as emerging data shows pro-thrombotic complications occurring. Many of these agents are weight based and require lab monitoring for efficacy and safety. Unfortunately, obesity is an ongoing epidemic in the US as well and potentially worse outcomes in those with COVID-19. It is imperative that pharmacists have an understanding on how the pharmacokinetics can be altered in patients that are obese or large body weight, along with the recommended dosing weight to use.

While it is not uncommon to occasionally have adult-sized patients in children’s hospitals, some are preparing to support the care of more adult patients in the event of COVID-19 surge in their region. This means that pharmacists who traditionally only care pediatric patients will be caring for adult patients without conditions that are associated with pediatrics diseases. Due to the COVID-19 pandemic, our pharmacotherapy clinic had to rapidly create new workflows for telehealth services to allow for continued care of our patients, while minimizing their possible exposure to COVID-19. We will be discussing how we have rapidly converted our face-to-face pharmacist visits to telehealth visits to minimize patient exposure in the ambulatory care clinics, as well as, pharmacist/provider exposure to patients unnecessarily. In addition, we will discuss the types of telehealth visits provided, the roadblocks we have encountered/lessons learned and current reimbursement for such services.

CAR-T cell therapies have the potential to transform cancer treatment. Their unique toxicities require rapid recognition and specialized treatment. Collaboration between multiple disciplines is needed for the optimal management of patients undergoing CAR-T cell therapy. The purpose of this podcast is to discuss key issues that non-oncology practitioners in acute care settings will need to be familiar with to appropriately treat patients whom underwent CAR-T cell treatment and are experiencing adverse effects related to treatment.
