Safety is at the forefront of all processes in the pharmacy department. ASHP has been a champion of safety since the organization originated more than 75 years ago.
ASHP received funding from the Food and Drug Administration’s (FDA) Safe Use Initiative to develop and implement national standardized concentrations for IV and oral liquid medications. This is the first national inter-professional (physicians, pharmacists, nurses) initiative to standardize medication concentrations with the goal of reducing prescribing, dispensing, and medication errors for patients of all ages in settings ranging from hospital to home. This outstanding program, called Standardize 4 Safety, will have substantial downstream effects on safety, cost, efficiency, and error-reduction.
Standardization will result in consolidated/converged product demand from hospitals, and more instances of ready-for-use drug preparations from manufacturers, simpler drug libraries, improvements to automatic drug recognition by infusion pumps, and improved clinical information system data exchange.

Within a health system, student pharmacists on rotation often are responsible for making numerous clinical interventions. However, oftentimes students are not engaged in the clinical intervention documentation, and do not have the opportunity to practice and develop their documentation skills prior to going into independent practice as a pharmacist. In this episode of Educator Essentials, we talk about implementing a standardized method for tracking student pharmacist interventions in the inpatient setting.

Medication excipients (often-referred to as inactive ingredients) provide several functions, such as serving as binders, disintegrants, solubilizers, preservatives, and/or assisting with pH adjustment. However, medication excipients may cause adverse events or allergic reactions in some patients. Several regulatory and technological gaps exist that hinder practitioners from preventing adverse events and allergic reactions related to these excipients.
This podcast is intended to bring awareness to current regulatory and technological gaps associated with managing medication excipient allergies and intolerances, highlight common operational challenges involved with identifying and preventing medication excipient allergic reactions, and provide current “best practices” to address these issues.
